comprehensive
community-based primary
health care, mental health
and social services for
diverse populations in
underserved communities
throughout New York City.
Community Healthcare Network
is looking for a Family
Planning Educator to work
primarily in our
Williamsburg/Bushwick CABS
Health Center. The Family
Planning Educator is
responsible for educating
and counseling family
planning patients in the
health center, as well as
organizing and facilitating
workshops in schools and
community-based
organizations in the
Williamsburg/Bushwick
neighborhood. Additionally,
the educator will work with
an evaluator to evaluate the
clinic based education that
is provided. The educator is
also responsible for keeping
the clinic stocked with
educational materials,
organizing health events,
and representing CHN at
community-wide events.
Specific responsibilities
include:
* Provide group and
individual education to
patients on topics related
to reproductive health and
document in the medical
record as necessary.
* Assist with development of
curriculum for educational
workshops.
* Assist with development
and implementation of
evaluation tools for
individual and group
education.
* Document educational
efforts and maintain records
of all activities.
* Complete monthly reports.
* Work with Director of
Health Education to create
and order appropriate
educational materials.
* Provide reproductive
health education and
outreach in the community
that is served by the
assigned health center.
* Facilitate related
services for health center
patients and CHN staff as
appropriate, with respect to
their confidentiality and
privacy.
* Organize and participate
in special events related to
reproductive health.
Requirements:
* Bachelor’s degree in
health education, nursing,
or health-related field, or
equivalent degree.
* Experience working in a
reproductive health setting.
* Experience working with
diverse communities.
* Demonstrated ability to
organize, develop,
implement, and evaluate
educational activities.
* Excellent verbal/written
communication skills.
* Strong presentation and
group facilitation skills.
* Interest in and conviction
for CHN mission.
* Bilingual
(English/Spanish)
